Job Description

The Speech Language Pathologist will provide high-quality speech therapy services to patients in a skilled nursing facility (SNF). Responsibilities will include assessments, treatment planning, and therapeutic interventions to improve communication and swallowing disorders in patients. The ideal candidate will be proficient in using Rehab Optima (Net Health) for documentation and patient management.

Responsibilities

• Conduct assessments and evaluations of patients’ speech and language capabilities.
• Develop and implement individualized treatment plans based on patient needs.
• Utilize Rehab Optima (Net Health) for documentation and therapy management.
• Work collaboratively with interdisciplinary teams to improve patient outcomes.
• Monitor patient progress and adjust treatment plans as necessary.
• Educate patients and families about speech and language disorders and therapy goals.

Required Experience

Minimum of 1 year of experience in a skilled nursing facility (SNF) setting.

Certifications/Licensure

Active California Speech Language Pathologist license required.
