Monday - Friday, Day Shift, 07:00 - 17:00.

Job Description

As a Registered Nurse in Home Health, you will provide individualized, goal-oriented nursing care in accordance with physician orders. Your role will include serving as a Case Manager for patients where you are the lead discipline, ensuring holistic care and support for individuals from various backgrounds.

Responsibilities

• Deliver personalized nursing care based on the nursing process.
• Coordinate and manage patient care as the lead Case Manager.
• Collaborate with healthcare professionals to optimize patient outcomes.
• Document care provided and maintain accurate medical records in compliance with regulations.

Required Experience

Experience in home health or hospice care preferred.

Certifications/Licensure

Current Registered Nurse (RN) licensure is required.
