Days: Monday - Friday (with weekend rotation approximately every 4th week)

This position is responsible for providing individualized goal-oriented nursing care using the nursing process in accordance with a physician’s order. In addition, this position serves as Case Manager in cases where this position is the lead discipline.

Responsibilities

• Deliver individualized nursing care in accordance with physician orders.
• Perform case management duties as the lead discipline when applicable.
• Utilize the nursing process to develop and implement patient care plans.
• Ensure holistic care that promotes health and well-being.

Required Experience / Certifications / Licensure

• Current RN license in the state of practice.
• Experience in home health or hospice preferred.
• Strong communication and interpersonal skills.
