Assignment Duration

This is a 13-week contract position.

Schedule/Shift

7.5 hrs/day, 37.5 hrs/week; Schedule: 8:00 AM – 4:00 PM; Rotating weekends: Every 4th or 5th weekend; Holidays: One summer and one winter holiday.

Job Description

The Discharge Planner is responsible for coordinating discharge planning and placement of patients in rehabilitation facilities. This role requires a strong understanding of hospital operations and the patient's journey post-acute care.

Responsibilities

• Facilitate discharge planning for acute inpatient hospital patients.
• Coordinate placement in rehabilitation facilities.
• Collaborate with medical and nursing staff to ensure continuity of care.
• Provide guidance to patients and families regarding discharge options.

Required Experience

• Minimum of 2 years of experience in acute inpatient hospital discharge planning.
• Experience with subacute patients is required.

Certifications/Licensure

Hospital experience required, including experience with discharge planning.
