Assignment Duration

This is a 13-week contract position.

Schedule/Shift

7.5 hrs/day, 37.5 hrs/week, 8:00 AM – 4:00 PM, rotating weekends (every 4th or 5th weekend), holidays (one summer and one winter holiday).

Job Description

The Discharge Planner will be responsible for managing the discharge process for patients, including coordinating placement in rehabilitation facilities. This role will involve working with subacute patients and ensuring a smooth transition to appropriate post-acute care.

Responsibilities

• Develop and implement discharge plans for patients.
• Coordinate with healthcare teams to facilitate patient placements.
• Assess and evaluate patients for subacute rehabilitation needs.
• Assist patients and families with understanding discharge instructions and follow-up care.

Required Experience

• Minimum of 2 years of experience with acute inpatient hospital discharge planning.
• Experience with subacute patients required.
• Hospital experience required, including experience with discharge planning and placement in rehabilitation facilities.

Certifications/Licensure

Current licensure as a nurse or relevant healthcare professional is required.
