Schedule/Shift

Hours: 8 hrs/day, 40 hrs/week
Schedule: 5 days per week, Day Shift (8a-4p)
Every other weekend with comp days during the week.

Job Description

The position includes performing patient assessments, developing care plans, ensuring appropriate utilization of services, and managing case responsibilities while demonstrating knowledge of hospice regulations. It also requires floating to different counties according to need and participating in interdisciplinary team meetings.
