Job Description

We are seeking a dedicated Registered Nurse to provide hospice care in Morris and Sussex Counties. The nurse will be responsible for patient assessments, developing care plans, ensuring the proper utilization of services, and maintaining compliance with hospice regulations and insurance guidelines. This role requires excellent communication with the interdisciplinary team.

Responsibilities

• Perform patient assessments to determine appropriateness for hospice care and develop care plans based on patient and family needs.
• Maintain visit productivity standards and demonstrate knowledge of insurance reimbursement.
• Complete discharge assessments and perform daily data transfers for patient records.
• Utilize age-appropriate teaching methods for patients and families; document teaching plans and tools used.
• Assume responsibility for case management, revising care plans as necessary and prioritizing care needs.
• Participate in interdisciplinary team (IDT) meetings and communicate patient progress regularly.
• Maintain a safe environment by following infection control measures and managing medications effectively.
• Attend monthly staff meetings and engage in performance improvement initiatives.
• Provide mentorship and orientation for new employees.

Required Experience

Experience in hospice care or a related field is preferred.

Certifications / Licensure

Current RN licensure is required.
