Job Description

We are seeking a Licensed Vocational Nurse (LVN) with recent home health experience to provide routine visits within the coverage area of Hinds County. Candidates may be asked to assist in other territories within the facility's service area, which is approximately a 60-mile radius from the facility address. Effective communication and a commitment to patient care are essential for this role.

Responsibilities

• Complete routine patient visits as assigned.
• Achieve 32 points per week through patient care activities.
• Document care and maintain accurate patient records using HomeCare HomeBase (HCHB) software.

Required Experience

Recent home health experience is required. HomeCare HomeBase experience is also required, though the client may consider candidates without it at a reduced rate during the initial training period.

Certifications / Licensure

Active LVN licensure is required.
