Job Description

The Outpatient Physical Therapist will evaluate assigned patients and develop, implement, and modify treatment plans according to the patients’ needs, goals, and physician direction. Responsibilities include delegating and directing all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs for effective return to community activities, and serving as an educator for patients, families, students, coworkers, and self. The therapist will also uphold the organization’s Mission Statement, Core Values, and Code of Conduct, ensuring compliance and confidentiality regarding patient and company information.

Responsibilities

• Complete comprehensive evaluations of assigned patients and develop appropriate treatment plans and goals.
• Document medical records clearly, concisely, and accurately, ensuring compliance with state, federal, and insurance guidelines.
• Participate in student program activities as assigned, providing a positive learning environment.
• Actively pursue professional growth through continuing education and involvement in professional organizations.
• Maintain professional licensure in good standing and adhere to the discipline-specific State Practice Act and regulatory agencies.
• Demonstrate awareness of performance improvement methodologies and engage in organizational initiatives.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• A minimum of 1 year of outpatient clinical experience is required.
• Current CPR certification.
• Active and applicable state licensure for Physical Therapy.
