Monday - Friday, 5 × 8 hour shifts.

Job Description

The Home Health Physical Therapist will provide high-quality care in a home health setting, focusing on evaluations, treatment plans, and therapy interventions. This role requires experience with OASIS documentation and the ability to manage a productive caseload, averaging 4-6 visits per day within specified geographical areas. The role may involve travel within a 60-mile radius of the facility.

Responsibilities

• Conduct patient evaluations and develop treatment plans.
• Implement therapy interventions and monitor patient progress.
• Document care plans and patient encounters using Homecare Homebase.
• Collaborate with other healthcare professionals in case conferences.
• Ensure compliance with all regulatory and agency standards.

Required Experience / Certifications / Licensure

• Licensed Physical Therapist with prior experience in home health.
• Experience with OASIS required.
• Homecare Homebase documentation experience preferred; training may be available.
