This is a 13-week contract position.

Schedule

Shift: Day 5x8-Hour (07:00 - 16:30), with potential for 4x10 scheduling. Weekend work required every other weekend or 4 weekend days within a 4-week schedule.

Job Description

The Case Manager RN will be responsible for care coordination and discharge planning within an acute hospital setting. This role requires effective communication with patients and interdisciplinary teams to enhance patient outcomes and ensure compliance with health regulations.

Responsibilities

• Conduct needs assessments and develop patient care plans.
• Ensure compliance with medical necessity per evidence-based guidelines.
• Perform pre-certification review and prior authorizations.
• Coordinate with various departments including social services, pharmacy, and physical therapy.
• Review continued stay and discharge plans, collaborating with healthcare providers to optimize patient outcomes.

Required Experience / Certifications / Licensure

• Active Alaska RN license required for auto offer.
• Certification in BLS preferred.
• ACM certification preferred.
• Minimum of 1 year acute care experience required, with Epic charting experience.
• No local candidates accepted; must reside at least 50 miles from the facility.
