4x10 hour shifts (07:00 - 17:30), weekends required every other weekend or 4 weekend days in a 4-week schedule.

Job Description

The Case Manager RN plays a vital role in ensuring quality patient care and effective discharge planning. The ideal candidate will have a minimum of one year of acute care experience and be proficient in using the Epic charting system. This position requires collaboration with interdisciplinary teams to coordinate care and navigate the complex healthcare environment, ensuring compliance with regulations and standards.

Responsibilities

• Conduct assessments and develop case management plans based on patient needs.
• Coordinate care transitions and ensure safe discharge for patients.
• Utilize evidence-based guidelines to determine medical necessity and appropriateness of care.
• Engage with patients and their families to provide education and resources.
• Collaborate with interdisciplinary teams to facilitate comprehensive patient care.
• Maintain compliance with CMS guidelines and Joint Commission standards.

Required Experience / Certifications / Licensure

• Current Alaska RN license required.
• BLS certification required; ACM certification preferred.
• Minimum of 1 year of acute care experience is essential.
• Epic experience mandatory.
• Experience with case management and care coordination is preferred.
