Schedule

Day shift: 4x10 hours, from 07:00 to 17:30 with weekend requirements every other weekend.

Job Description

We are seeking a dedicated Case Manager RN to join our team. This position involves coordinating patient care, ensuring compliance with health regulations, and engaging in discharge planning for various patient populations. The ideal candidate will have acute care experience and a strong background in case management.

Responsibilities

• Manage care coordination and discharge planning for patients.
• Perform assessments and determine medical necessity as per guidelines.
• Utilize Epic EMR for documentation and patient management.
• Work collaboratively with interdisciplinary teams to meet patient needs.
• Participate in quality improvement and compliance with health regulations.

Required Experience / Certifications / Licensure

• Current AK RN license is required.
• BLS certification preferred.
• ACM certification preferred.
• Minimum of 1 year of acute care experience required.
• Epic EMR experience is mandatory.
