Day 4x10-Hour shifts (07:00 - 17:30), every other weekend required.

Job Description

We are seeking a skilled Case Manager RN with an acute care background to join our team. The ideal candidate must have an active Alaska RN license and experience with Epic charting. This position requires strong skills in care coordination, discharge planning, and compliance with CMS guidelines.

Responsibilities

• Coordinate patient care plans and discharge processes.
• Utilize Epic for patient documentation and charting.
• Conduct Continued Stay Reviews and determine medical necessity according to evidence-based guidelines.
• Work collaboratively with interdisciplinary teams including social services, pharmacy, and health services.
• Ensure compliance with HIPAA and The Joint Commission standards.

Required Experience / Certifications / Licensure

• Active Alaska RN license required (license in hand for auto offer).
• 1 year of acute care experience.
• BLS certification preferred.
• ACM certification preferred.
• Epic experience required.
