Care Manager - Acute Care

Job Description

The Care Manager will be responsible for performing care coordination and discharge planning within an acute care facility. The role requires strong communication skills, the ability to assess patient needs, and the development of effective care plans aligned with established criteria.

Responsibilities

• Care coordination and discharge planning for patients.
• Collaborate with interdisciplinary teams to enhance patient outcomes.
• Conduct needs assessments and determine medical necessity per evidence-based guidelines.
• Ensure compliance with healthcare regulations such as HIPAA and CMS standards.
• Manage patient caseload effectively, typically up to 25 patients.

Required Experience / Certifications / Licensure

• Minimum of 2 years acute medical care management experience.
• Experience in care coordination and discharge planning is essential.
• Current CA state license required (pending license accepted if obtained by start date).
• Knowledge of charting systems, specifically Epic, is required.
• Ability to work weekends as needed, typically one full weekend every four weeks.
