Acute Care Case Manager

Job Description

This is a 13-week contract position. The role requires a seasoned Acute Care Case Manager with expertise in case management and discharge planning for an acute care facility. The selected candidate will be responsible for ensuring effective care coordination and facilitating optimal patient outcomes.

Schedule

Day Shift: 5x8-Hour (08:00 - 16:30)

Responsibilities

• Conduct comprehensive assessments and develop individualized care plans.
• Manage and coordinate patient care transitions, including discharge planning.
• Facilitate family meetings and contribute to the treatment planning process.
• Utilize knowledge of HMO and IPA systems to ensure benefits eligibility.
• Coordinate with interdisciplinary teams, including physical therapy and social services.
• Arrange for DME, SNF, and ambulance transports as needed.

Required Experience / Certifications / Licensure

• Minimum 2 years of experience in Acute Care Case Management.
• Must hold a valid California state license; pending licenses accepted if licensed upon start date.
• Experience with CMS regulations and The Joint Commission standards.
• Prior experience managing patient ratios of 1:25 during day shifts.
