ARMStaffing is looking for a Licensed Clinical Social Worker, Case Management in Laguna, California.

This is a 13-week contract position.

Schedule/Shift

Variable 5-day workweek with department operations occurring 7 days per week. Days off will vary and are not guaranteed to be back-to-back.

Job Description

This position is for an acute care facility focused on case management and discharge planning for adult patients, with a caseload of approximately 20-23 patients. The Licensed Clinical Social Worker will work collaboratively with medical teams, insurance case managers, and other staff to ensure safe and efficient patient transitions to the lowest level of care. The charting system used is Epic.

Responsibilities

• Conduct initial assessments within 24 hours of patient admission.
• Develop and coordinate individualized discharge plans.
• Facilitate continuity of care by coordinating post-acute care needs.
• Work closely with a multidisciplinary team, including RN's and MSW/LCSWs.
• Document care delivery and patient progress accurately.

Required Experience

• 1 year of acute care experience required.
• Experience with case management and discharge planning preferred.
• Familiarity with Epic charting system is required.

Certifications/Licensure

• Current BLS certification required.
• LCSW degree required.
