Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist will evaluate assigned patients and develop, implement, and modify treatment plans in accordance with patient needs, goals, and physician direction. The therapist is responsible for delegating and directing all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs that facilitate an effective return to community activities. This role includes serving as an educator to patients, families, students, and coworkers.

Responsibilities

• Complete comprehensive evaluations of assigned patients and utilize clinical reasoning to develop appropriate treatment plans focused on progressing patients towards discharge.
• Document medical records clearly, accurately, and timely, ensuring compliance with state, federal, and clinic policies.
• Participate in student programs, providing a positive learning environment with adequate supervision.
• Create and actively pursue a professional development plan, engaging in continuing education and professional activities.
• Adhere to the discipline-specific State Practice Act and maintain professional licensure in good standing.
• Understand and contribute to the organization’s performance improvement initiatives.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• Minimum of 1 year of outpatient clinical experience.
• Current CPR certification.
• Active license for the state where work will be performed.

Schedule

This position offers a 5x8.0 shift schedule.

This is a 13-week contract position.
