Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist will evaluate all assigned patients and develop, implement, and modify treatment plans based on patient needs, goals, and physician direction. This role is responsible for the delegation and direction of all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs for effective community reintegration. The therapist will also serve as an educator for patients, families, students, coworkers, and themselves, while adhering to professional standards and organizations' core values.

Responsibilities

• Complete comprehensive evaluations of assigned patients and establish appropriate treatment plans and goals.
• Document medical records in a clear, concise, accurate, and timely manner, ensuring compliance with state and federal guidelines.
• Participate in student programs, providing a positive learning environment and supervising students as needed.
• Create individualized professional development plans and pursue continuing education opportunities.
• Maintain professional licensure in good standing and adhere to ethical standards of conduct.
• Engage in performance improvement initiatives to enhance quality of care.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience preferred.
• CPR certification required.
• Active and applicable state license for practice.

This is a 13-week contract position.
