5x8.0-811107, flexible 40 hours per week with 2 evenings until 7 PM.

Job Description

The Outpatient Physical Therapist will evaluate assigned patients and develop, implement, and modify treatment plans according to the patient’s needs, goals, and physician direction. Responsibilities include delegating and directing all discipline-specific therapy services and ensuring the design of effective treatment programs aimed at facilitating return to community activities. The therapist will also serve as an educator for patients, families, students, and coworkers while maintaining compliance with organizational policies and the Code of Conduct.

Responsibilities

• Conduct comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop treatment plans aimed at patient discharge within an established length of stay.
• Complete medical record documentation in a timely and accurate manner.
• Provide care under signed, valid physician orders.
• Participate in student programs by providing supervision and feedback.
• Engage in professional development and maintain licensure in good standing.
• Adhere to the organization’s performance improvement initiatives to enhance service quality.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ years of outpatient clinical experience.
• Current CPR certification.
