5x8-Hour Day Shift (08:00 - 16:30), with weekend requirements to work every other weekend.

Job Description

We are seeking an experienced Case Manager for an acute care setting in Eureka, California. This position is ideal for nature lovers, situated close to various state and national parks. The Case Manager will be instrumental in care coordination, discharge planning, and ensuring eligibility for benefits.

Responsibilities

• Coordinate patient care and streamline discharge planning to enhance patient outcomes.
• Conduct needs assessments and facilitate necessary services like prior authorizations and worker’s compensation case management.
• Navigate complex healthcare challenges in compliance with CMS and HIPAA guidelines.
• Collaborate with interdisciplinary teams, including social services, to ensure comprehensive care delivery.

Required Experience / Certifications / Licensure

• Must have at least 2 years of acute care experience.
• Relevant certifications in case management preferred.
• Experience with HMOs, IPAs, protective services process, advance directives, and POLST.
• Active state license in hand at time of submission.
