Day shift; 5x8-Hour schedule from 08:00 - 16:30.

Job Description

We are seeking a seasoned Acute Care Case Manager to join our team. The ideal candidate will have at least 2 years of experience in acute care case management or discharge planning, with a focus on coordinating care for a diverse patient population including neonates, adolescents, adults, and geriatrics. You will be responsible for developing treatment plans, participating in family meetings, and ensuring seamless care transitions for patients.

Responsibilities

• Assess patients and coordinate care effectively.
• Develop and implement discharge plans.
• Organize and participate in family meetings.
• Coordinate with interdisciplinary teams for comprehensive patient support.
• Manage DME and SNF arrangements independently.
• Utilize knowledge of various healthcare programs and patient populations.

Required Experience / Certifications / Licensure

• Must have hospital experience; behavioral health experience does not apply.
• Minimum 2 years of Acute Care Case Management/Discharge Planning experience.
• California state license required; pending license accepted but must be licensed upon start date.
• Familiarity with Epic charting system preferred.
• Must be willing to work every other weekend to meet department needs.
