Part-Time Licensed Practical Nurse (LPN)

Assignment Duration

This is a 13-week contract position.

Schedule

Part-time position with 2-3 shifts rotational, including weekends and holidays. All contracts must run through the holiday season and start on a Tuesday or Thursday for full day orientation.

Job Description

The LPN will provide skilled nursing care for residents, ensuring a comprehensive approach tailored to the age-specific needs of adult and geriatric patients. The role requires collaborating effectively with nursing staff, physicians, and ancillary staff while adhering to defined policies and procedures under the supervision of a Registered Nurse.

Responsibilities

• Collect and assess data regarding the resident's physical and psychosocial condition, communicating findings to the RN and interdisciplinary team.
• Contribute to developing and maintaining culturally relevant care plans that reflect nursing diagnoses and interventions to achieve desired outcomes.
• Evaluate resident outcomes and recommend updates to care plans while ensuring comprehensive care solutions are addressed.
• Participate in mentoring and orientation activities for new staff and maintain current knowledge through ongoing education.
• Provide leadership under the RN's guidance while considering safety and costs in care delivery.

Required Experience / Certifications / Licensure

• Current NY LPN licensure.
• BLS certification (American Heart Association or RQI).
• One year of experience serving as an LPN in a long-term care facility.
