Ambulatory Registered Nurse (RN) Case Manager

Job Description

The Ambulatory Registered Nurse (RN) Case Manager for Primary Care provides comprehensive, patient-centered nursing care in an outpatient clinic setting, supporting individuals and families across the continuum of care. This role utilizes the nursing process to assess, plan, implement, and evaluate care; administer medications and treatments; perform clinical procedures; and provide patient education and care coordination in collaboration with providers and interdisciplinary teams.

Responsibilities

• Perform comprehensive patient assessments and develop individualized plans of care.
• Implement and evaluate nursing interventions and patient responses.
• Administer medications and treatments safely and accurately per provider orders and policy.
• Recognize changes in patient condition and initiate appropriate interventions or escalation of care.
• Provide patient and family education related to care, medications, and discharge needs.
• Collaborate with providers and interdisciplinary team members to coordinate care.
• Facilitate referrals, follow-up care, and continuity of services.
• Document assessments, interventions, and outcomes accurately and timely in the electronic health record.
• Participate in quality and performance improvement activities.

Required Experience / Certifications / Licensure

• Current full, unrestricted Registered Nursing license in Alaska or other U.S. state required.
• Case Management experience in a clinic or hospital setting required within the last 2 years.
• BLS certification required; Case Management certification preferred.
• Graduate of an accredited school of nursing; Bachelor’s degree preferred.
• High school diploma or equivalent required.
