Monday to Thursday: 8 AM - 5 PM; Friday: 8 AM - 2 PM

Job Description

The Outpatient Physical Therapist is responsible for evaluating assigned patients and developing, implementing, and modifying treatment plans based on patient needs, goals, and physician direction. This role requires ample interaction with patients and includes the delegation and direction of various therapy services to achieve effective rehabilitation outcomes.

Responsibilities

• Conduct comprehensive evaluations of assigned patients.
• Develop and implement individualized treatment plans focused on rehabilitation.
• Complete medical record documentation accurately and in a timely manner.
• Ensure care is provided under valid physician orders.
• Participate in student program and provide mentorship as needed.
• Maintain professional licensure and adhere to continuing education requirements.
• Support organizational performance improvement initiatives.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience required.
• Current CPR certification.
• Active state licensure for Physical Therapy.
• Experience in orthopedics is required.
