Outpatient Physical Therapist

Job Description

We are seeking a dedicated Outpatient Physical Therapist to evaluate and treat patients in accordance with their needs and goals. The successful candidate will develop, implement, and modify treatment plans under physician direction, ensuring effective return to community activities. As an educator for patients, families, and coworkers, you will play a pivotal role in promoting patient care and safety within our organization.

Responsibilities

• Complete comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop treatment plans and goals.
• Document patient care clearly, accurately, and in a timely manner.
• Ensure care is delivered under signed, valid physician orders.
• Participate in student program offering guidance and evaluations.
• Engage in professional development through continuing education and self-study.
• Adhere to the discipline-specific State Practice Act and maintain professional licensure.
• Contribute to performance improvement initiatives and provide feedback as necessary.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited institution.
• Minimum of 6 months to 1+ year of outpatient clinical experience.
• Current CPR certification.

Schedule

This position is scheduled for a 5x8.0 shift, starting November 23rd, with 60-minute evaluations and follow-ups every 30 minutes.
