5x8.0 shift, starting November 30th, 40 hours/week.

Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans based on the patients' needs, goals, and physician direction. The therapist is responsible for delegating and directing all discipline-specific therapy services for these patients, ensuring the design of functional treatment programs that allow for effective return to community activities. This role includes serving as an educator for patients, families, students, and coworkers.

Responsibilities

• Completes comprehensive evaluations of assigned patients and utilizes clinical reasoning to develop appropriate treatment plans and goals.
• Ensures clear, concise, accurate, and timely documentation in compliance with state, federal, and organizational guidelines.
• Participates in the student program, providing a positive learning environment with appropriate supervision.
• Actively pursues professional development and applies learned information to meet the organizational objectives.
• Adheres to the discipline-specific State Practice Act and maintains professional licensure in good standing.
• Implements performance improvement initiatives to enhance the quality of services provided.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 6 months to 1+ year of outpatient clinical experience.
• Current CPR certification.
• Active and applicable license for the state where work will be performed.
