Job Description
The Physical Therapist provides therapy services to patients referred, including assessment, treatment plan development and implementation, follow-up, and discharge planning. This role actively assists with the clinical aspects of the department, including program development, department operations, and performance improvement activities. The Physical Therapist supervises and coordinates the Physical Therapy and Physical Therapy Assistant student programs and oversees Licensed Physical Therapy Assistants, Rehab Technicians, and Volunteers.
Responsibilities
- Perform comprehensive diagnostic evaluations to determine the status of the physical and cognitive abilities of inpatients and outpatients.
- Develop and implement individualized treatment plans with patient/family participation, considering specific needs and measurable goals.
- Adhere to documentation policies, including initial patient evaluations, daily and weekly progress notes, discharge summaries, and other required documentation.
- Participate in patient care conferences, family conferences, in-service programs, and departmental meetings.
- Counsel patients and family members regarding evaluation, care, and treatment.
- Initiate and complete discharge planning in a timely manner and provide effective education and training.
- Assist with the coordination and supervision of the Physical Therapy and Physical Therapy Assistant student programs as well as the volunteer program.
- Promote adherence to departmental productivity standards and participate in performance improvement activities.
- Maintain an awareness of community resources to make appropriate referrals.
- Identify and provide educational opportunities for self, the department, and the community.
- Demonstrate competence in performing assigned patient care responsibilities considering age and developmental specific needs.
- Foster and promote positive and professional relationships with medical staff and referral sources.
Required Experience / Certifications / Licensure
- Bachelor’s, Master’s, or entry-level Doctorate of Physical Therapy degree from an accredited college or university.
- Current New Mexico Physical Therapist licensure.
- BLS certification from AHA required.
- No previous experience required.
