Schedule

Shift hours are as follows:

• M: 6:30 AM - 12:30 PM
• T/Th: 9:00 AM - 7:00 PM
• W: 6:30 AM - 3:00 PM
• F: 7:00 AM - 3:30 PM

Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans according to the patients' needs, goals, and physician directions. Responsibilities include delegation and direction of all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs for effective return to community activities, and serving as an educator for patients, families, students, coworkers, and self.

Responsibilities

• Conduct comprehensive evaluations of assigned patients and develop individualized treatment plans.
• Document patient care accurately and timely in compliance with state and federal guidelines.
• Provide educational support to students and ensure a positive learning environment.
• Maintain professional licensure in good standing and complete continuing education requirements.
• Participate in organizational performance improvement initiatives.
• Ensure adherence to safety protocols and patient handling guidelines.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• Minimum of 6 months outpatient clinical experience required.
• Current CPR certification.
• Active and applicable state licensure required.
• Prior experience with the agency is highly desired.
