Acute Care Case Manager

Job Description

We are seeking an experienced Acute Care Case Manager to join our team. This is a 13-week contract position with a focus on care coordination and discharge planning within an acute care setting. The ideal candidate will have prior hospital experience and a strong background in case management.

Responsibilities

• Conduct assessments and develop treatment plans for patients.
• Coordinate care and discharge for patients and their families.
• Organize and participate in family meetings.
• Work collaboratively with interdisciplinary teams for effective care delivery.
• Ensure compliance with CMS regulations and The Joint Commission standards.
• Manage prior authorizations and arrange DME, SNF, and ambulance transport as needed.

Required Experience / Certifications / Licensure

• Minimum of 2 years of acute care case management or discharge planning experience required.
• Must have a current CA state license (pending licenses accepted but must be licensed upon start date).
• Experience with electronic medical records, specifically Epic, is preferred.
• Knowledge of care coordination, admission criteria, and disease management.

Schedule

This position involves a day shift, working 5 days a week with shifts from 08:00 AM - 04:30 PM. The candidate is required to work every other weekend.
