Care Manager - Acute Care Facility

This is a 13-week contract position.

Schedule

Day shift, 5x8-hour schedule (08:00 AM - 04:30 PM). Typically one full weekend every four weeks, subject to department needs.

Job Description

We are seeking an experienced Care Manager to perform care coordination and discharge planning in an acute care setting. The ideal candidate will exhibit strong skills in patient assessment, needs assessment, and ensuring the medical necessity of care aligned with evidence-based guidelines.

Responsibilities

• Coordinate care for patients and oversee discharge planning processes.
• Evaluate patient needs and develop individualized plans of care.
• Ensure compliance with CMS and Department of Health regulations.
• Manage patient caseloads of up to 25 individuals.
• Conduct needs assessments and order durable medical equipment as necessary.
• Work effectively with interdisciplinary teams and various healthcare providers.

Required Experience / Certifications / Licensure

• Minimum of 2 years of acute medical care management experience.
• Experience with care coordination and discharge planning.
• Current California state license required; pending licenses accepted but must be licensed by start date.
• Proficiency in Epic charting system is preferred.
• Must commit to working 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day).
