RN Case Manager

Job Description

The RN Case Manager coordinates management of care to ensure optimum resource utilization, service delivery, and compliance with external review agencies. This role includes providing ongoing support and expertise through comprehensive assessment, care planning, implementation, and evaluation of individual patient needs. The RN Case Manager enhances the quality of patient management and satisfaction, promoting continuity of care and cost-effectiveness by integrating case management functions, utilization review, and discharge planning. The role involves acting as a patient advocate through resource utilization, discharge planning, and addressing the holistic needs of the patients.

Responsibilities

• Coordinates management of patient care and resources.
• Implements care plans and evaluates individual patient needs.
• Facilitates discharge planning and ensures follow-up care to enhance patient outcomes.
• Advocates for patients by addressing their holistic needs.
• Collaborates effectively with interdisciplinary teams (IDT).
• Utilizes clinical knowledge to enhance patient satisfaction and care continuity.

Required Experience / Certifications / Licensure

• Minimum of 2 years recent RN Case Management experience.
• Experience in a Long-Term Acute Care (LTAC) or Short-Term Acute Care Hospital (STACH) setting.
• MCG, discharge planning experience across acute care settings.
• Strong collaboration with interdisciplinary teams.
• Current California RN license.
• BLS certification from the American Heart Association.

Schedule

Monday - Friday, 8:30 AM - 5:00 PM

Assignment Duration

This is a 13-week contract position.
