Job Description

The RN Case Management unit is responsible for managing patient care in an acute setting, focusing on coordination, collaboration, and communication among healthcare providers to ensure optimal patient outcomes.

Responsibilities

• Manage patient care plans and coordinate services across healthcare providers.
• Work collaboratively with a multidisciplinary team, including social workers.
• Monitor patient progress and adjust care plans as necessary.

Required Experience / Certifications / Licensure

• Must be a licensed Registered Nurse (RN) with experience in an acute care setting.
• Basic Life Support (BLS) certification required.
• ACLS certification is preferred.
