8-hour shifts, Monday to Friday, from 8:00 AM to 4:30 PM.

Job Description

The Hospice Registered Nurse (RN) will provide care for terminally ill patients in their homes or nursing facilities, managing cases effectively while ensuring comfort and quality of life for patients and their families. The RN will work collaboratively within a small team, addressing the needs of a manageable patient load.

Responsibilities

• Conduct patient assessments and develop individualized care plans.
• Administer medications and perform medical procedures as necessary.
• Provide education and emotional support to patients and their families.
• Coordinate and communicate with interdisciplinary teams for comprehensive care.
• Document patient interactions and progress using the EPIC electronic medical record system.

Required Experience / Certifications / Licensure

• Minimum of 1 year of hospice experience is required.
• Current West Virginia RN license in good standing.
• Basic Life Support (BLS) certification (AHA).
