Physical Therapist

Job Description

This is a 13-week contract position. The Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with patient needs and physician direction. This role is responsible for the delegation and direction of all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs that facilitate an effective return to community activities. The Physical Therapist serves as an educator for patients, families, students, coworkers, and self, while performing other professional duties associated with patient care and therapy service development.

Responsibilities

• Completes comprehensive evaluations of assigned patients.
• Utilizes clinical reasoning to create appropriate treatment plans and goals aimed at progressing patients towards discharge.
• Collaborates with interdisciplinary teams to address barriers to discharge, including timely equipment trialing and patient training.
• Manages individual, concurrent, and group treatment sessions based on patient care needs.
• Documents medical records clearly, accurately, and timely, adhering to state, federal, and organizational guidelines.
• Participates in student programs, offering a positive learning environment and supervision.
• Actively pursues professional development and applies learned information for organizational objectives.
• Engages in patient and professional advocacy efforts through community events and memberships.
• Maintains professional licensure in good standing and completes state-mandated continuing education requirements.
• Understands performance improvement methodologies and contributes to quality service enhancement.

Required Experience / Certifications / Licensure

• BLS Certification required before the first shift.
• Valid, current State license required.
• Minimum of 2 years of experience required.
• Previous LTACH experience preferred.
