This is a 13-week contract position.

Job Description

The Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with patient needs and physician direction. This role includes the delegation and supervision of all discipline-specific therapy services for those assigned patients, ensuring functional treatment programs that facilitate effective community reintegration.

Responsibilities

• Conduct comprehensive evaluations of assigned patients and develop appropriate treatment plans and goals.
• Collaborate with an interdisciplinary team to address barriers to patient discharge.
• Manage individual, concurrent, and group treatment sessions based on patient care needs.
• Complete accurate and timely medical record documentation in compliance with guidelines.
• Participate in student program activities, providing supervision and evaluations as necessary.
• Develop a personalized professional growth plan and actively pursue continuing education opportunities.
• Engage in patient and professional advocacy efforts, including community events and fundraising activities.
• Adhere to the State Practice Act and maintain professional licensure in good standing.
• Contribute to organizational performance improvement initiatives to enhance service quality.

Required Experience / Certifications / Licensure

• BLS Certification required prior to the first shift.
• Valid and current state license as a Physical Therapist.
• A minimum of 2 years of experience in a physical therapy role.
• Previous experience in Long-Term Acute Care Hospitals (LTACH) is preferred.
