Physical Therapist - Home Health

Job Description

Dartmouth Health Home Care provides exceptional nursing, rehabilitation, hospice, and personal care services across more than 70 towns. As a Physical Therapist, you will play a vital role in assessing and treating patients in their homes, collaborating with an interdisciplinary team to deliver high-quality care.

Responsibilities

• Evaluate patients' needs for physical therapy and develop tailored treatment plans.
• Implement treatment programs in accordance with agency policies and professional standards.
• Maintain clear communication with healthcare team members and patients regarding treatment plans.
• Document evaluations, treatment goals, and objectives accurately.
• Educate patients and family members on treatment rationale and procedures.

Required Experience / Certifications / Licensure

• Must hold an unencumbered Physical Therapist license in Vermont and New Hampshire or a compact license.
• Graduate of an accredited Physical Therapy program.
• One year of experience as a Physical Therapist in a home health care setting is preferred.

Schedule

This position offers day shifts with guaranteed hours of 36 per week.
