Job Description

We are seeking a dedicated Physical Therapist to provide in-home therapy services aimed at maximizing the independence and safety of our home care and hospice patients. The successful candidate will work independently while also being an integral part of our multidisciplinary team.

Responsibilities

• Provide in-home therapy services to patients in accordance with their individualized care plans.
• Maximize patient independence and safety through therapeutic interventions.
• Collaborate with a multidisciplinary team to ensure comprehensive patient care.
• Document patient progress and adjust treatment plans as necessary.

Required Experience / Certifications / Licensure

• Current Vermont licensure as a Physical Therapist.
• Valid driver’s license and reliable vehicle required.
• Experience in a home care setting is preferred.
