Job Description

Looking for a dedicated Registered Nurse with an active California license and experience in a Skilled Nursing Facility (SNF) setting. Ideal candidates will have knowledge of the Rehab Optima (NetHealth) computer system and be prepared to provide high-quality care to patients.

Responsibilities

• Administer medications and treatments as prescribed.
• Conduct patient evaluations and develop care plans.
• Monitor patient progress and adjust care plans accordingly.
• Collaborate with interdisciplinary teams to enhance patient care.
• Document care activities and update patient records in the Rehab Optima system.

Required Experience / Certifications / Licensure

• Active RN license in California.
• Experience in a Skilled Nursing Facility (SNF).
• Familiarity with the Rehab Optima (NetHealth) computer system.
