Job Description

The skilled nursing and rehabilitation facility is seeking qualified candidates to provide top-notch care in their 81-bed facility. Candidates should ideally have experience in a skilled nursing facility setting.

Responsibilities

• Deliver patient-centered care in a skilled nursing environment.
• Collaborate with a multidisciplinary team to ensure quality rehabilitation services.
• Monitor patient progress and provide updates to the medical team.
• Implement care plans tailored to individual patient needs.

Required Experience / Certifications / Licensure

At least one 13-week assignment in a skilled nursing facility is preferred. Candidates must provide three references from their three most recent employers.
