Monday to Friday, 8 AM to 5 PM with holiday coverage as needed.

Job Description

The Home Health and Hospice Registered Nurse will be responsible for providing high-quality care to patients in their home settings. The nurse must be adaptable and able to manage changes to schedules effectively, demonstrating a commitment to patient-centered care.

Responsibilities

• Conduct comprehensive assessments and develop individualized care plans using OASIS guidelines.
• Deliver nursing care, including medication administration and case management.
• Provide education and support to patients and their families regarding care plans and health management.
• Travel to various patient homes for visits, demonstrating flexibility in managing schedule changes.
• Document patient care and progress in the HealthWyse EMR system.

Required Experience / Certifications / Licensure

• Must have previous experience in home health and hospice care.
• Experience with OASIS documentation is essential.
• Must possess a valid RN license.
• Ability to manage and adjust to schedule changes while traveling between patient homes.
