Job Description

The Physical Therapist is responsible for evaluating, planning, directing, and administering Physical Therapy services as prescribed by a licensed Provider in inpatient, outpatient, and home health settings. The focus is on restoring function, improving mobility, relieving pain, and preventing or limiting physical disabilities to enhance patient independence and overall functioning.

Responsibilities

• Treat patients using Physical Therapy modalities and skilled therapy techniques, instructing on adaptive/assistive equipment.
• Supervise and direct Physical Therapy Assistants, maintaining communication with the Rehab team and consulting other departments for collaborative outcomes.
• Perform assessments on all patients related to physician orders and interpret results within licensure scope.
• Design treatment programs for diverse disabilities and medical issues, aligning with accepted practice patterns and physician orders.
• Formulate realistic short and long-term treatment goals and involve patients/families in care planning.
• Monitor and evaluate treatment effects, recommending changes to physicians as necessary.
• Ensure accurate and timely patient charges and documentation, in compliance with regulatory standards.
• Contribute to departmental budget information related to Physical Therapy needs for optimal patient care.
• Comply with safety procedures to ensure a safe environment for patients, visitors, and staff.
• Participate in performance improvement activities and undertake other assigned duties.

Required Experience / Certifications / Licensure

• Current IL Physical Therapist License
• Valid Driver's license and proof of personal auto insurance
• BLS certification
• Preferred: APTA Membership
• Minimum of 1 year of experience in Physical Therapy.
