Day Shift: 5 days per week, 8-hour shifts (08:00 - 17:00).

Job Description

We are seeking an experienced Home Health Registered Nurse to provide skilled nursing care to patients in their homes. The ideal candidate will possess a strong background in patient assessments, care planning, and medication management while ensuring compliance with safety and care standards.

Responsibilities

• Conduct comprehensive patient assessments and document findings.
• Develop and implement individualized care plans based on patient needs.
• Administer medications and monitor patient responses.
• Educate patients and families regarding care plans and health management.
• Utilize various adaptive devices and assistive equipment as needed.
• Ensure patient safety and injury prevention during home visits.
• Maintain accurate documentation of patient care and services provided.
• Collaborate with an interdisciplinary team for optimal patient outcomes.

Required Experience / Certifications / Licensure

• Minimum 1 year of nursing experience, preferably in home health care.
• State Registered Nurse license or compact license required.
• Basic Life Support (BLS) certification required.
• OASIS training preferred.
• Valid driver's license and reliable transportation for home visits.
• COVID-19 vaccination or exemption form on file, as well as a flu shot.
