Job Description

We are seeking a qualified Speech-Language Pathologist to join our home health team. The ideal candidate will provide high-quality care to patients in their homes, utilizing best practices for assessments, treatments, and patient education.

Responsibilities

• Conduct evaluations and develop individualized treatment plans for home health patients.
• Provide therapy for communication and swallowing disorders in adults and geriatrics.
• Complete OASIS assessments and documentation accurately and timely.
• Work collaboratively with interdisciplinary team members, including physical and occupational therapists.
• Maintain productive patient ratios and manage a case load efficiently.

Required Experience / Certifications / Licensure

• Minimum of 1 year of skilled home health experience required.
• Current BCLS certification required.
• OASIS experience is mandatory; proficiency in EPIC is preferred.
• Candidates must possess reliable transportation for patient visits.
• COVID-19 primary series vaccination and one booster shot required.
