Job Description

We are seeking a qualified Home Health Speech-Language Pathologist (SLP) to provide exceptional care to patients in their homes. This position requires an active Oregon license and entails visiting up to 6 patients per day with a focus on achieving high productivity standards.

Responsibilities

• Conduct evaluations and assessments for speech and language disorders.
• Develop and implement individualized treatment plans.
• Provide patient and family education regarding communication and swallowing disorders.
• Optional: Participate in interdisciplinary team meetings to discuss patient progress.

Required Experience / Certifications / Licensure

• Minimum 1 year of skilled home health experience required.
• Oregon Speech-Language Pathologist license required.
• BCLS certification required.
• Familiarity with Oasis documentation is a plus.
