Shift: 7:30 AM - 4:00 PM, Every third weekend and holidays required.

Job Description

The RN Care Manager is responsible for discharge planning and care coordination within the Care Management department. This role involves collaboration with various healthcare professionals to ensure optimal patient care and resource management.

Responsibilities

• Conduct discharge planning for patients.
• Coordinate care across multiple disciplines.
• Address patient needs and facilitate efficient patient transitions.
• Maintain communication with patients and healthcare team members.

Required Experience / Certifications / Licensure

• Minimum of 5 years of nursing experience.
• Active RN license.
• Experience in care management is preferred.
• Flexibility to float to other units as needed.
