Job Description

We are seeking an experienced RN Care Manager to join our Care Management team. The ideal candidate will possess expertise in discharge planning and care coordination, providing essential support to patients in their transition to home or other care settings.

Responsibilities

• Coordinate discharge planning and patient care transitions.
• Collaborate with multidisciplinary teams to ensure comprehensive patient care.
• Provide case management support as needed.
• Manage patient caseload effectively, maintaining a ratio of 1:20-24.
• Utilize electronic medical records (EMR) for patient documentation and communication.
• Support occasional weekend coverage as required.

Required Experience / Certifications / Licensure

• Must hold a current RN license.
• Minimum of 5 years of nursing experience, with a focus on care management preferred.
• Experience with discharge planning and care coordination essential.
• Flexibility and adaptability to cover various needs within the care management department.
