Part-Time Licensed Practical Nurse (LPN)

Job Description

The Licensed Practical Nurse (LPN) is responsible for providing both direct and indirect skilled nursing care for assigned residents, ensuring the delivery of care that meets the age-specific needs of adult and geriatric patients. Under the supervision of a Registered Nurse, the LPN will follow established policies and procedures while maintaining collaborative relationships with other healthcare professionals.

Responsibilities

• Collect and assess data relevant to residents' physical and psychosocial conditions, communicating findings to the RN and interdisciplinary team.
• Contribute to the development of culturally relevant care plans, engaging residents and families in the process.
• Recommend revisions to care plans based on evaluations of resident outcomes and identify needs for referrals.
• Maintain knowledge and competency in nursing practice through ongoing education and mentoring of staff.
• Provide leadership and act as a role model in professional settings, prioritizing safety and cost-effectiveness in care delivery.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure
• BLS Certification (AHA or RQI specifically)
• Minimum of 1 year experience serving as an LPN, preferably in a long-term care facility

Schedule

This part-time position requires the LPN to work a combination of 2 eight-hour shifts one week and 3 eight-hour shifts the next week, including every other weekend and holidays.
