Licensed Practical Nurse (LPN) - Full Time Nights

Full Time Nights, 5x8 per week. Weekends and holidays are required.

Job Description

The Licensed Practical Nurse (LPN) will provide both direct and indirect skilled nursing care for assigned residents. The LPN must demonstrate the knowledge and skills necessary to deliver age-appropriate care to adult and geriatric patient populations. The responsibilities include collaborating with the healthcare team to ensure the effective delivery of care, maintaining clinical competency, and participating in the development of individualized care plans.

Responsibilities

• Collect data relating to the physical and psychosocial conditions of residents continuously, and communicate findings to Registered Nurse (RN) and interdisciplinary team members.
• Assist in creating culturally relevant and developmentally appropriate care plans that reflect nursing diagnoses and interventions.
• Recommend revisions to care plans based on evaluations of resident outcomes, ensuring comprehensive care coverage.
• Participate in the mentoring and orientation of staff, promoting a safe and cost-effective care environment.
• Maintain knowledge and competency in nursing practice through continuing education and training.

Required Experience / Certifications / Licensure

• Current New York LPN Licensure
• BLS certification through AHA or RQI
• Minimum of 1 year experience as an LPN in a Long-Term Care (LTC) facility
