Registered Nurse - Skilled Nursing Facility

Job Description

We are seeking a dedicated Registered Nurse (RN) with active CA license and experience in Skilled Nursing Facilities (SNFs). The ideal candidate will possess knowledge of the Rehab Optima (NetHealth) computer system. This position requires a commitment to providing high-quality nursing care to patients in a rehabilitation setting.

Responsibilities

• Administer nursing care to patients in accordance with their care plans
• Utilize the Rehab Optima (NetHealth) system for documentation and care management
• Collaborate with multidisciplinary teams to ensure patient-focused care
• Monitor patient progress and adjust care plans as necessary
• Educate patients and families about health management and wellness

Required Experience / Certifications / Licensure

• Active California RN License
• Experience in a Skilled Nursing Facility (SNF)
• Knowledge of Rehab Optima (NetHealth) computer system

Schedule

This is a day shift position, offering guaranteed hours.
